What Is Vaginal Rejuvenation?

Vaginal rejuvenation can mean many different things. The most common meaning is surgery to tighten the vaginal canal when it feels too loose—a condition called vaginal laxity. This happens when the internal levator muscles (also called Kegel muscles or puborectalis muscles) become widely separated, most commonly due to vaginal childbirth, though it can occur in women who have never had children.

Surgical Vaginal Tightening: The Vaginoplasty Procedure

The most effective treatment for vaginal laxity is surgery called a vaginoplasty. At Pelosi Medical Center, we perform comprehensive vaginoplasty procedures that include multiple components—something that requires highly developed skills and experience. Not all surgeons perform this procedure as thoroughly as we do. Our approach includes:

  • Perineoplasty Component: Surgical tightening of the skin and muscles at the vaginal opening, included with all our vaginoplasty procedures.

  • Rectocele and Pelvic Floor Reconstruction Component: Repair of a bulge in the posterior vaginal wall by reconstructing the supports. Most women have some degree of rectocele, and if not addressed, the vaginoplasty might fail.

  • Levatorplasty Component: Surgical tightening of the internal vaginal muscles using specialized suturing techniques. This creates the tightening effect. Our advanced suturing technique avoids lumpy knots inside the vaginal walls, resulting in a stronger, more precise repair, faster healing, and more comfortable intimacy for you and your partner.

  • Colporrhaphy Component: Surgical tightening of the vaginal canal skin performed with finesse. Too much skin removal causes pain; too little creates lumpy scarring. Our refined techniques avoid both problems.

Labia Majora Rejuvenation Options

Some women are bothered by labia majora that appear too flat, too wrinkled, or too saggy. These concerns can arise at any age. Treatments may be surgical, nonsurgical, or a combination of both:

  • Fat Injections to correct deflation or wrinkles

  • Platelet Rich Plasma (PRP) Injections to supplement fat injections

  • Amniotic Fluid Injections to supplement fat injections

  • Filler Injections with Juvederm or Restylane to add volume

  • Nonsurgical Laser Treatments to tighten loose skin with Fractional CO2 Laser (FemiLift or MonaLisa)

  • Nonsurgical Radiofrequency Treatments to tighten loose skin with Viveve or ThermiVa

  • Labia Majoraplasty Surgical Reduction to correct loose or wrinkled skin

Vaginal Opening Treatments

The vaginal opening (also called the vaginal introitus) can be either too loose or too tight, each requiring different approaches.

When the Vaginal Opening Is Too Loose:

This is caused by a thinned-out perineum. A normal perineum is thick with muscle tissue across the middle. Surgery called a perineoplasty is the only effective fix.

When the Vaginal Opening Is Too Tight:

Sex can be painful or impossible. Treatment depends on the underlying cause:

  • Botox Therapy for tightness caused by muscle tension (with or without previous surgery)

  • Dilator Therapy for muscle tension-related tightness

  • Hormonal Therapy (cream, suppository, or other forms) for menopausal dryness and atrophy

  • Nonsurgical Laser Therapy with FemiLift or MonaLisa Fractional CO2 Laser for menopausal women who are too tight or dry and prefer non-hormonal options

  • Nonsurgical Radiofrequency (RF) Therapy with Viveve or ThermiVa. There are slight differences between RF and laser; the right choice is individualized after discussion and examination.

  • Surgical Therapy with Reverse Perineoplasty for tightness caused by previous surgery when dilator therapy hasn’t helped

  • Platelet Rich Plasma (PRP) and Amniotic Fluid Injections sometimes combined with these therapies to enhance healing

Non-Surgical Vaginal Canal Treatments

If you have mild vaginal laxity, you might achieve temporary improvement with nonsurgical energy-based treatments using Viveve Radiofrequency (RF) or FemiLift Fractional CO2 Laser. However, the effect won’t be as strong as surgery because these devices only tighten skin, not the underlying muscles.

For vaginal dryness and pain caused by menopausal atrophy, most women achieve excellent results with hormonal therapy, energy-based devices (Viveve or FemiLift), or both. These treatments work much more reliably for improving lubrication than for tightening.

What About Urinary Leakage?

Urinary leakage is a medical problem typically covered by insurance. The management of urinary incontinence is not considered vaginal rejuvenation. Various types of urinary leakage exist, with treatments ranging from surgical to nonsurgical or combined approaches:

  • Bladder Lift Surgery with either a sling procedure or non-sling procedure (Burch procedure is most common)

  • Nonsurgical Bladder Support with a Pessary—a soft silicone ring inserted intravaginally for support when needed

  • Pelvic Floor Muscle Therapy with a pelvic floor physical therapist and/or biofeedback devices, including structured Kegel exercise programs

  • Pharmacological Therapy with hormones or medications that stabilize an overactive bladder

Energy-based devices (Viveve, FemiLift, MonaLisa) are being used for urinary incontinence, though they aren’t covered by insurance. While these treatments help some women, there’s no reliable way to predict effectiveness beforehand. Understanding this limitation is important before proceeding with these options.
